About the Susquehanna Wellness Clinic
The Susquehanna Wellness Clinic is a primary care office with locations in Frenchville and Clearfield, PA. The Frenchville facility has roots in the local community as the building formerly served as the Susquehanna Rural Free Clinic. The Clearfield location formerly held the offices for PennDOT's Engineering District 2.
The clinic is operated by Mature Resources, a subsidiary of the Clearfield County Area Agency on Aging.
At SWC, patients are the center of our focus. The clinic serves adult patients 18 and older with an emphasis on providing quality and meaningful care. We take a comprehensive approach to care by providing chronic care management, telehealth and telemedicine services as well as onsite health and wellness programs. Other services we offer include patient transportation to elders to medical appointments, in home services to patients physically unable to come into the clinic, and remote visits to senior centers and high rises. Additionally, SWC participates in most managed care plans, processes most insurances and is able to accept credit card and cash payments.
