If your insurance carrier sends a denial stating "not a covered service," there are many things to consider. Some insurances do not cover certain services. Our staff will assist as best as we can, but your insurance carrier may need to be contacted for further information.

SWC will bill your account for fees incured during your appointment. The amount due can be accessed through our Patient Portal. If you need a plan for paying off your fees, contact our office and our staff will assist you in creating a personalized plan.

WHEN CAN I USE TELEHEALTH SERVICES?

Telehealth services are designed for appointments that do not reqiure a physical examination. Appointments should be used when a patient needs to contact their health care provider about personal health information, lab results, prescription management or follow-up care that may not need an in-person exam.​ Established patients can schedule a Telehealth appointment through our OnPatient (https://www.onpatient.com)portal or by calling (tel:(814)765-2695)our office.

HOW CAN I MAKE AN APPOINTMENT?

Individuals can schedule appointments online through our Patient Portal (javascript:void(0))or by giving us a call. Plan to arrive at least 15 minutes before your scheduled appointment, and try to schedule routine appointments with as much advance notice as possible. Although we can make same-day appointments for urgent care needs, it is best to call ahead to let the office know you will be coming in.

WHAT SHOULD I BRING TO MY APPOINTMENT?

You will need to bring your insurance card and picture identification.  It is always a good measure to keep an updated list of your medications to bring along, but at the first visit it is wise to bring your medications in the correct bottles with you. It is also important to bring with you all of the over the counter medications you take as well as any herbal medications/ remedies.

WHAT SHOULD I DO IF I NEED TO CANCEL MY APPOINTMENT?

First of all, don't worry! We understand that sometimes things happen and canceling is unavoidable. If you are unable to keep a scheduled appointment, please try to notify our office at least 24 hours in advance. Office staff will cancel the appointment and work with you in rescheduling for another date.

WHAT HAPPENS IF I MISS MY APPOINTMENT?

It is important to try and keep all scheduled appointments. When you schedule an appointment our office staff reserves time for you to be seen by a doctor and prepares your health records for the visit. We ask you to call our office as soon as you know you are unable to attend your appointment. Not only does this allow us to reschedule you for another time, but it also frees up time for another patient to be seen.

WHAT HAPPENS IF I AM LATE TO MY APPOINTMENT?

Please call our office to let us know you will be late to an appointment. Because several patients are scheduled each day, office staff can only wait so long before seeing the next scheduled patient. If you are more than 10 minutes late, the office may need to reschedule your appointment.

WHAT IF I AM HAVING A MEDICAL EMERGENCY OR NEED CARE AFTER HOURS?

If you are experiencing a life-threatening emergency, call 911 right away. Do not call your doctor first. For non-life-threatening urgent medical care after hours or on weekends, please go to the nearest hospital emergency room for services.

WHAT IS THE DIFFERENCE BETWEEN AN ANNUAL PHYSICAL AND AN OFFICE VISIT?

A preventive exam typically occurs every 1-3 years depending on your insurance coverage. It is charged based on your copay parameters and subject to your benefits and deductible (normally covered completely by your insurance). This is usually a longer visit and includes a full exam of the body. The provider will address any chronic issues, plans for care including medications and referrals, and recommendation of any preventive measure you should take. You can expect that all chronic medications will be renewed for one year during this annual exam. Throughout the year when you need refills you can contact your pharmacy directly as the prescription will be there for a year. When refills are out, that means it is time to come back in and see us! This preventive visit is also considered a wellness exam.  
If you bring specific, acute concerns to your preventive visit exam, you may incur a separate charge in addition to the preventive charge. 
Combining a preventive visit and office visit can be an effective way to help you manage your overall health. If you and your provider are not able to discuss all your health concerns during your visit, you may need to schedule a follow-up visit.  
Preventive Visit: 
Frequency: Every 1-3 years (depending on your insurance) 
Cost: Covered based on your insurance cover- age. If you discuss items outside of the defined benefit, charges may apply.

If you are of Medicare age, you have a free wellness visit that is limited in what it covers. We recommend an Annual Comprehensive Visit (ACV), which is also covered by Medicare, but is subject to your deductible.

SWC cannot accept walk-in patients. However, if an established patient calls in and same-day appointments are available, they can fill that appointment.
